Vishnyak Dental Clinic — Kyiv, Pozniaky

Questions to Dr. Dmytro Vishnyak

How many implants have you placed over your career?

Over my career I have placed more than 5,000 implants — about 300 per year on average. Documented survival rate in my practice is 96–97%, which matches the best international results for modern 7th-generation systems.

Which implant systems do you use?

I work with several systems for different clinical needs. Premium: Straumann BLX 7th generation, the most advanced available. Mid-tier: DIO and MegaGen (AnyOne and AnyRidge sublines — the choice depends on indications). Budget Korean option: Osstem. I use the American BioHorizons rarely — it is slightly behind in development today. I match the system to the clinical case and the patient's budget — I do not push a single brand.

What happens if the implant does not integrate?

This is extremely rare with modern implant systems, but medicine is not an exact science and rejections do happen. Because we stand behind our work, in such cases the entire replacement procedure is performed at no cost to the patient — therefore we are committed to delivering quality. I personally put my reputation on the line for that. There are two replacement scenarios: either wait 2–3 months for the socket to heal, or, if bone conditions allow, immediately place a new implant of a larger diameter.

What is the warranty on your work?

The warranty on the implant is 5 years, provided the patient follows the doctor's recommendations. In practice, if the patient maintains good oral hygiene and attends regular check-ups (with professional cleaning every 3–6 months and follow-up X-rays as needed), the warranty effectively extends to 10+ years.

What is the service life of an implant?

An implant has no expiry date — it can last a lifetime. It depends on the system chosen and the surgeon's expertise in the specific case. Technologies do not stand still, and doctors who consistently take advanced training courses, keep up with the times, and master modern techniques and approaches deliver better long-term outcomes. In our clinic we additionally augment the soft tissues around the implant — this acts as a barrier against microorganisms and preserves the surrounding bone, extending service life potentially to lifetime.

What is 3D navigation and why is it needed?

3D-printed surgical guides for precise implant positioning — essentially a navigation system. They are mandatory when working on edentulous jaws and when placing implants at angles of 25 degrees or more, as well as transsinus and into the pterygoid region — where positioning accuracy is critical. I consider this a must-have option for any clinic that takes implantology seriously. We use guides in every complex case.

How does the first consultation work?

During the consultation we take a detailed history and diagnostic scans — 3D panoramic, CT. If needed, lab tests, bacterial cultures and smears. In select cases — a complete blood count, ECG, and densitometry, which measures bone mineral density. The consultation is not just a conversation: I show similar clinical cases from practice and demonstrate how the solution applies to the patient's situation. I typically offer 2–3 treatment options. The patient leaves with a written treatment plan and financial estimate — which makes the decision much easier.

Which is better: an implant or a bridge?

An implant preserves the adjacent teeth (a bridge requires grinding them down) and prevents bone atrophy at the missing tooth site. A bridge is cheaper and faster but designed for 7–10 years; an implant lasts decades. For a single missing tooth an implant is the optimal choice; for multiple defects we compare the options at the consultation.

What happens to the jaw if a missing tooth is not replaced?

Bone tissue at an extraction site begins to atrophy within 3–6 months — this leads to dentoalveolar deformities. Adjacent teeth tilt toward the missing one, the bite is disturbed, and the opposing tooth drifts as well. The longer the delay, the more bone grafting will be needed for future implantation.

Is implant placement painful?

The placement itself is done under local anesthesia and is painless — most patients compare the discomfort to a routine tooth extraction. Post-op there is moderate pain and swelling for 2–3 days, controlled with standard pain relievers (Nimesil, Ibuprofen). For patients with dental anxiety, sedation is available.

What anesthesia is used for implantation?

The standard is local anesthesia (Ultracaine, Septanest) with a vasoconstrictor. Duration of action is 1.5–3 hours, which covers any routine procedure. For longer interventions (sinus lift, All-on-4/6) we use sedation — the patient is conscious but relaxed and free of anxiety.

How long does placing a single implant take?

A single implant — 15–20 minutes from anesthesia to suturing, including preparation and the 3D navigation guide. Complex cases (with sinus lift or bone grafting) — 1.5–3 hours. We assess the exact time from the CT scan before surgery and inform the patient in advance.

How many implants can be placed in one visit?

In good bone conditions — up to 16 implants in one visit. All-on-4 (4 implants per jaw) and All-on-6 (6 implants) are standard same-day protocols. Full rehabilitation of both jaws combined with bone augmentation, sinus lift and implantation is often split into 2 visits 2–4 weeks apart — this is easier on the patient and the tissues.

Can an implant be placed immediately after tooth extraction?

Yes, this is immediate implantation: the tooth is extracted and an implant is placed in the same socket. Saves 3–6 months. Suitable when there is no inflammation and bone density is sufficient. We make the decision based on the 3D scan BEFORE extraction — sometimes it is better to let the socket heal for 2–3 months.

What tests are needed before implantation?

3D panoramic scan and CT — mandatory. Complete blood count, coagulogram, fasting glucose. For chronic conditions — a clearance letter from the relevant specialist (endocrinologist, cardiologist). All tests can be done at any lab in Kyiv; results are valid for 1 month.

How long does an implant take to integrate?

The standard osseointegration period is 3–6 months. Faster on the lower jaw (3–4 months), longer on the upper jaw (4–6 months) due to different bone density. After integration the crown is placed. With 7th-generation systems (Straumann BLX) timelines shorten by 30–40%.

What is a gingival former and why is it needed?

It is a temporary titanium element screwed onto the implant during integration to shape the natural gum contour around the future crown. Worn for 2–4 weeks, then replaced with the permanent crown. This stage is important for aesthetics and hygiene — without it the gum margin can look unattractive.

What is All-on-4 and who is All-on-6 for?

All-on-4 is a full-jaw restoration protocol on 4 implants with a fixed non-removable prosthesis. Used in full edentulism and average bone density. All-on-6 uses 6 implants for greater stability and is used in significant bone atrophy or high chewing loads. We make the choice based on the CT.

When is a sinus lift needed?

A sinus lift raises the floor of the maxillary sinus to create room for an implant in the upper jaw when the patient's own bone is less than 6–8 mm. Open sinus lift — for bone deficit of 4 mm or less; closed — for 6–8 mm. Often performed simultaneously with implantation. We prefer autografts (the patient's own bone) over synthetic materials.

What is bone grafting and is it always needed?

Bone grafting is augmentation of bone tissue at the future implant site. It is needed in roughly 30–40% of cases of long-term tooth loss. In our clinic we prefer autografts — the patient's own bone (harvested from the lower jaw ramus). Documented success rate ~95–96%, ahead of synthetic substitutes.

How much does a turnkey implant cost in Kyiv?

The price in Kyiv in 2026 depends on the implant system and case complexity. Turnkey (implant + abutment + crown): Korean systems (Osstem, DIO) — from ~25,000 UAH, mid-tier (MegaGen AnyOne/AnyRidge) — from ~30,000 UAH, premium Straumann BLX — from ~50,000 UAH. The precise estimate comes after the consultation and CT.

What determines the price of an implant?

Main factors: implant manufacturer (premium systems cost more), case complexity (whether bone grafting or sinus lift is needed), crown material (PFM vs zirconia), timeline. Any add-ons are locked in the written estimate BEFORE treatment starts — no surprises along the way.

What is included in the turnkey implant price?

A standard turnkey package includes: 3D navigation guide when needed, the implant with screw, the abutment (connecting element), a temporary crown during integration, the permanent crown (zirconia or E.max), all surgical procedures and materials. NOT typically included: initial consultation with CT, sinus lift, bone grafting — these are separate line items quoted in the estimate in advance.

How much does All-on-4 cost in Kyiv?

All-on-4 (4 implants per jaw + fixed prosthesis) — from ~120,000 UAH with Korean systems and from ~250,000 UAH with premium Straumann. The price includes surgery, implantation, and both temporary and permanent fixed prosthesis. The choice between 4 or 6 implants is individual and based on the CT.

How much does All-on-6 cost?

All-on-6 — from ~180,000 UAH (Korean systems) up to ~350,000 UAH (Straumann). More expensive than All-on-4 because of two additional implants and a more complex surgical protocol. Used in significant bone atrophy or high chewing loads.

Does insurance cover implants?

In Ukraine in 2026, virtually no voluntary health insurance (DMS — Parasol, UNIQA, etc.) covers implantation — it is classified as a reconstructive procedure. For international patients, dental implants are also typically not covered by standard travel or health insurance — please check with your provider in advance. For domestic patients, partial reimbursement is possible via the Ukrainian income tax deduction (using receipts and the treatment contract); we issue the required paperwork.

Can implantation be paid in installments?

Yes. For domestic patients we work with installment plans from PrivatBank and Monobank — 6, 12 and 24 months without interest. Internal installments are also possible: payment by treatment stage (after surgery, after crown placement). For international patients we agree on a stage-based payment schedule. Terms are discussed at the financial consultation.

Who should not get implants?

Absolute contraindications: uncontrolled diabetes, active oncological disease, severe bleeding disorders, immunodeficiency, acute psychiatric conditions, age under 18. There is no upper age limit — we have patients successfully treated in their 70s and 80s.

Can people with diabetes get implants?

With well-controlled diabetes (stable glucose levels, clearance from an endocrinologist) — yes, with additional healing monitoring and more frequent visits. Survival rates may be slightly lower than in the general population. With uncontrolled diabetes, implantation is postponed until blood sugar is stabilized.

Can smokers get implants?

Yes, but rejection risk is 2–3 times higher in smokers. Nicotine constricts blood vessels and slows bone regeneration. Minimum: stop smoking 1 week before surgery and 4–6 weeks after. Ideally — quit completely. At the consultation we discuss this individually based on your smoking history and daily count.

Can implants be placed with periodontitis?

With active periodontitis, implantation is not performed — the periodontitis itself must be treated first (professional cleaning, curettage, surgery if needed). After the disease is stabilized, implantation is possible, with particular emphasis on augmenting the soft tissues around the implants as a microbial barrier.

What should I avoid in the first week after implantation?

First 7–10 days: no smoking, no alcohol, no hot or hard foods on the surgical side, no rinsing on the first day, no heavy exercise, no flying (if possible), sleep with an elevated pillow for the first 2–3 nights. Brush teeth on the operated side with a soft brush starting 24 hours after surgery.

How long does swelling last after implantation?

Swelling peaks on day 2–3 after surgery, then begins to subside. It clears fully within 5–7 days. Apply ice during the first 24 hours (15-minute intervals) — this significantly reduces swelling. If there is severe facial asymmetry or increasing swelling after day 3 — call us, that is not normal.

Can I smoke after implantation?

Minimum 4–6 weeks — no. Nicotine reduces blood supply to gums and bone, slows implant integration by 30–50%, and raises rejection risk. If quitting completely is not possible — cut down as much as possible in the first 2 months and rinse with antiseptic after each cigarette.

How much does wisdom tooth extraction cost in Kyiv?

Simple extraction of an erupted wisdom tooth — from 1,600 UAH. Complex extraction of an impacted or dystopic tooth — 2,000–5,000 UAH. The price depends on the root position (per 3D scan), the need for bone sectioning and the number of fragments. Includes anesthesia, suturing, and a follow-up examination in 5–7 days.

How much do ceramic veneers cost per tooth?

E.max veneers (lithium disilicate) in Kyiv — from 12,000 to 35,000 UAH per tooth, depending on the lab and case complexity. Service life is 10–20 years with proper care. Premium segment — BiloGrad Lab (for aesthetic cases); we also work with mid-tier labs. We choose based on the patient's expectations and budget.

Zirconia or PFM crown — which to choose?

For front teeth — only zirconia or E.max: PFM (porcelain-fused-to-metal) shows a bluish margin at the gum line and looks unnatural. For molars both options are acceptable: zirconia lasts longer (15+ years) and is hypoallergenic; PFM is cheaper (4,500–9,000 UAH vs 10,000–12,000 UAH for zirconia). On implants — zirconia constructions on titanium frames.
